Rates and Enrolment
Enrolling for HealthSure™ is easy. There are no medical history questions to answer and no health exam is required. Below is the table of monthly rates which is followed by our ‘Easy Enrollment Form’ which only takes a few minutes to complete:
Note: Your monthly HealthSure™ Plan rates will increase as you reach each age band. If you are applying for couple or family coverage your monthly rates are based on the primary insured person.
Once you have completed the fillable PDF, email a signed copy with any necessary attachments to kathys@accessbenefits.ca and we will take it from there!
